Superior Health Plan-Centene is Hiring a Service Coordinator II Near Wichita Falls, TX

Note: Applicants must reside in the state of Texas and be able to travel to the following Texas service delivery area(s): Amarillo, TX, Abilene, TX, Del Rio, TX, Dumas, TX, Pecos, TX, Carrizo, TXEagle Pass, TX, Junction, TX, Segovia, TX, Odessa, TX, San Angelo, TX, or Wichita Falls, TX or surrounding areas

Position Purpose: Determine needs and eligibility of members for various services through field-based collaboration with medical, behavioral, and social support staff. Conduct screening and risk assessment interventions per program guidelines.

  • Conduct in-home face to face member assessments and telephonic outreach
  • Document assessments and contacts, develop and approve service plans, and ensure authorizations are completed
  • Conduct follow-up contacts or in-person visits to determine if services meet member’s needs
  • Assess member’s medical, safety and support service needs and coordinate the delivery of services
  • Provide education related to member’s health condition and care
  • Assist with modification and management of care plans under the direction of Registered Nurse in conjunction with member, medical consenter or caregiver, Department of Family and Protective Services caseworker and family, if applicable, as well as the managing physician
  • Maintain case record documents and enter information in an automated system, establishing a record for each member receiving services
  • Coordinate member care through collaboration with community outreach and member advocates, assigned service coordinators, and/or case managers
  • Educate providers and community resources on program components and available support services
  • Educate members to foster compliance with program and positively impact outcomes
  • 75% travel required
  • Performs other duties as assigned
  • Complies with all policies and standards

Education/Experience: Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field, or equivalent experience. 2 years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions. Other state specific requirements may apply.

Licenses/Certifications: LPN, LVN, LBSW, LMSW, LCSW, LPC or RN preferred, but not required. Current state driver’s license is required

Additional Preferred Qualifications:

  • Bachelor’s degree or higher in one of the following: Human Services, such as Psychology, Special Education, Rehabilitation Counseling, Social Work, Nursing, Occupational Therapy, Physical Therapy, Speech Lanaguage Pathology or Audiology is preferred.
  • 2 years’ of experience working with people with Intellectual and Developmental Disabilities or related conditions
  • 2 years of experience coordinating and managing community mental health resources for Medicaid recipients
  • Excellent communication and conflict resolution skills
  • Strong organizational and time management skills
  • Experience in field-based case coordination/case management roles – this roles to meet IN-PESRON with members in assigned service delivery area(s)
  • Strong clinical documentation skills and be proficient in using different computer programs to research information is required.
  • Proficient computer skills, knowledge, and experience working within large databases or provider information systems and Microsoft Office applications and the ability to use multiple avenues of communication (e.g. instant messaging, email, phone, video conferencing)

Comfortable using digital video conferencing platforms (Zoom, MS Teams) is required.

Pay Range: $25.97 - $46.68 per hour

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law. Total compensation may also include additional forms of incentives.
