Full Time (40 Hour Weeks)

The Transplant Coordinator RN is responsible for coordinating clinical aspects of organ transplantation as applied to the transplant recipient. These activities will include those involved in the evaluation, work-up and pre-transplant and/or post-transplant medical and surgical care of potential recipients. The coordinator will be actively involved in the development of the program and act as a liaison to referring physicians and dialysis centers. This individual will have responsibility for educational administrative and technical tasks related to clinical transplantation. The Transplant Coordinator is expected to participate in 24-hour on call (from home) coverage. Assisting in maintaining records (written and computerized) and providing information necessary for complete patient care is an important aspect of this position.

  • Coordinates all patient care needs before, during and after transplantation.

  • Organizes general transplant clinic.

  • Meets with follow-up patients in clinic; ad hoc with physician supervision.

  • Interviews new patients and patients on return visits and provides necessary data for office records, flow sheets, UNOS reporting forms and other active information systems maintained by the data coordinator.

  • Coordinates admission and operative planning for transplant candidates for whom an organ has been identified.

  • Supervises and coordinates all aspects of pre-transplant work-up of patients and potential living donors. Includes screening and selection process as well as coordinating work-up to expedite medical, social and insurance clearances for both recipients and donors.

  • Rotates coordinator patient call for patient care.

  • Facilitates communication between transplant clinic and nursing units (e.g., protocol/procedure changes, medical management updates, new treatments and medications, etc.).

  • Acts as liaison between referring physicians, dialysis units, and the organ transplant program in order to facilitate patient workups and keeps referring centers and physicians involved in our program.

  • Ensures that problems that are related to patient care, which arise related to referring centers or individuals, are immediately addressed in a satisfactory manner whether directly or in conjunction with the director.

  • May be asked to coordinate some transplant program conferences.

  • Coordinates new patient evaluation conferences for weekly review of candidates. Responsible for presenting completed data for review and discussion by the entire team.

  • Organizes weekly patient review conferences; review of important data, difficult cases and interesting cases from the previous week by the entire transplant team.

  • Coordinates patient service activities and record keeping.

  • Oversees some clerical personnel. Assists as necessary in scheduling procedures, admissions and outpatient appointments.

  • Assists data coordinator and director in preparing and maintaining clinical and statistical reports for the program. These reports may relate to regional and national networks as well as program tracking of workups and outcome data and clinical research protocols.

  • Oversees new patient listing and maintains relations with the Organ Procurement Agency.

  • Coordinates hospital activity related to patient care (e.g., discharge teaching of all transplant patients).

  • Coordinates operating room activity related to transplant services. Includes protocol development and data collection required during operative cases.

  • Coordinates hospital resources which affect patient welfare (e.g., initiates referrals to appropriate departments such as home health care, social work etc.).

  • Assures the rapid and accurate transmission of all information to outside hospitals/facilities which become involved in the care of transplant patients.

  • Participates in educational and professional activities.

  • Conducts, organizes and participates in programs for professional education, including inservice training for nursing personnel caring for transplant patients.

  • Participates in and/or conducts public education and public relations programs related to organ transplantation in order to increase public awareness of transplantation.

  • Acts to advise the director on ethical, legal and social issues as they relate to program development.

  • Participates in at least one national or international educational meeting related to the field of transplantation and/or the North American Transplant Coordinator’s Organization annually.

  • Directly involved in all protocol evaluations which pertain to patient care.

Required Qualifications:

  • Bachelor's Degree in Nursing degree (BSN) from an accredited school of nursing.

  • Applicants without a BSN degree must actively pursue and attain a BSN within five years of hire. Continued employment by Swedish beyond five years is contingent upon attaining the BSN degree. Registered Nurses employed by Swedish prior to December 1, 2012 are encouraged to pursue the BSN degree, but obtaining the degree will not be a condition of employment.

  • Washington Registered Nurse License upon hire.

  • Experience with or willingness to master the use of the transplant database.

Preferred Qualifications:

  • Master's Degree in Nursing

  • Certified Clinical Transplant Coordinator upon hire (without certification must actively pursue and attain certification within 2 years of hire)

  • 2 years of experience in the field of organ transplantation or critical care nursing.
