DescriptionTHE ROLEThe Program Director provides business development leadership for the implementation of strategic business development ventures and service line development for the North Division, including but not limited to strategic planning, capital planning, acquisitions, and collaborative joint ventures with other providers and partners. This position will also develop business plans and other strategic planning material as required to support business decisions, program development and implementation. The incumbent provides leadership and develops annual goals and objectives and is responsible for the strategic planning process of the North Division. The Director will also provide strategic expertise/counsel for the Executive Leadership of the Division and give valuable input and support to various Divisional and System groups, committees, initiatives and projects.
The position oversees the strategic market growth of all service lines within the Division to ensure seamless patient management along the care continuum offering cost efficient and high-quality services that are competitive in the marketplace. Duties include developing outreach program, cultivating partnerships with external partners within the local post acute industry, guiding strategic plans consistent with Divisional strategic plan, conducting annual goal setting, performance monitoring, physician relationship strategies, and performance improvement initiatives.
This position is accountable for assessing, planning, developing and implementing strategies and tactics to achieve optimal financial performance, cost efficiency, clinical superiority, and market leadership.
Essential Functions
  • Develops a collaboration plan with external community partners within the local post-acute care industry to foster relationships, facilitate appropriate care transitions, and to stay connected to industry standards and regulations.
  • Ensures alignment with community and agency groups to support patient care across the continuum to include gap assessment and analysis of future needs.
  • Serves as community partnership liaison for post-acute care placement; develops and leads internal process for prioritization of patient placement across all care sites within PSJHS.
  • Provides leadership and expertise to develop and oversee a system process of care transitions within post-acute care that delivers safe and effective transitions of care for patients and appropriate utilization of all segments of the health system.
  • Creates single-payor agreements with community partners across the healthcare landscape in conjunction with internal and external stakeholders.
  • Responsible for optimizing discharge planning methods and processes in partnership with Case Management teams.
  • Directs the identification and deployment of best practices to support organizational strategic objectives.
  • Drives the adoption of innovative practices.
  • Communicates with Directors of Case Management across the Population Health Area on key initiatives to drive quality performance.
  • Provides subject matter consultation to operating units in support of care management and social work goals.
  • Provides leadership and oversight for effective fiscal management, performance improvement activities, and compliance with all relevant policies/procedures and regulations. Actively participates in the budget and goal setting process.
  • Directs human resource initiatives and talent management efforts for designated areas. Provides leadership, guidance, development, and continuing education to staff. Selects, develops, coaches, mentors, and assesses performance of staff.
  • Active participant and member of the Executive Leadership team by furthering achievement of strategies, goals and objectives and reporting systems: Actively and willingly participates in assigned committees, task forces and other work groups. Attend management/retreats and/or other management functions. Participates in management skills development.
  • Manages all strategic requests that involve healthcare entities in the community that improve the care continuum.
  • Conduct routine service line portfolio review and makes overall strategic recommendations to ensure potential growth aspirations can be achieved on an annual basis as well as ensure a balanced market performance position.
  • Ensure that targets are developed to improve post-acute venue availability and utilization as appropriate.
  • Participate in PH&S committees and projects to ensure System best practices are shared and incorporated into service line strategies and structures.
  • Ensure that service line specific plans are created, reviewed, and implemented per plan expectations.
  • Performs other duties and responsibilities as assigned.
Qualifications
  • Bachelor's Degree in Health Services, Business Administration, Public Health, Nursing, or related field of experience
  • Master's Degree in Health Services, Business Administration, Public Health, Nursing, or related field of experience
  • 8 years of relevant experience (business development in healthcare, post-acute care, people leadership, etc.)
  • Understands and contributes to the development of key organizational priorities in the area of physician alignment, partnership strategies and payor models
  • Manages relationships across various teams within highly complex and matrixed organization
  • Anticipates and takes personal responsibility for customer satisfaction
  • Consistently goes out of the way to listen, understand and support the needs of others in a sensitive manner
  • Performs challenging tasks efficiently and effectively
  • Anticipates problems and takes corrective action prior to completing the task
