Founded in 1974, Swords to Plowshares is a community-based not-for-profit organization that provides counseling and case management, employment and training, housing, and legal assistance to veterans in the San Francisco Bay Area. We promote and protect the rights of veterans through advocacy, public education, and partnerships with local, state, and national entities. Believing that war causes wounds and suffering that last beyond the battlefield, our mission is to heal the wounds of war and to restore dignity, hope, and self-sufficiency to all veterans in need, and to end homelessness and poverty among those we serve. Summary (description)*
The Outreach Case Manager will be responsible for leading and supervising a team of outreach specialists, along with providing mobile case management services to enroll veterans into services and connect with internal and external providers within the community setting. Your primary responsibilities will involve overseeing the day-to-day operations of outreach activities, providing guidance and support to team members, enrolling, and referring veterans to services, and collaborating with internal and external community providers to ensure the success of outreach efforts.
Coordinate outreach activities and schedules to maximize coverage and impact, ensuring that outreach staff have the necessary resources and support to carry out their responsibilities effectively.
Provide outreach case management services to identify and enroll veterans who may benefit from our services within the community setting.
Connect clients with appropriate internal and external resources and support services, including housing assistance, healthcare, employment opportunities, education, and other social services.
Collaborate with internal team members and external partners to coordinate services and follow-up with veterans to identify any barriers hindering them from accessing services.
Timely submission of outreach efforts and veteran services into our outreach tracking system and Exponent Case Management (ECM).
Train, supervise, and mentor a team of outreach specialists, including setting performance expectations, providing regular feedback, and conducting performance evaluations.
Monitor program staff documentation to ensure all outreach efforts and veteran services are documented in our outreach tracking system and Exponent Case Management (ECM).
Safely transport veterans from the community to SF Service Center to enroll in services.
Establish and maintain positive relationships with community organizations, government agencies, and other stakeholders to expand outreach efforts and access additional resources.
Represent the organization at community events, meetings, and forums to raise awareness of services, gather feedback, and build relationships with key stakeholders.
Perform other related duties as requested by management.
