Responsibilities

• Serve as a primary point of contact for clients, providing support and guidance throughout their mental health journey.
• Conduct initial intakes and consultations to identify clients’ needs, preferences, and goals for treatment.
• Collaborate with clients and their families to develop personalized care plans that address their unique needs and promote holistic well-being.
• Coordinate referrals to appropriate mental health providers and community resources based on clients’ clinical needs and preferences.
• Monitor clients’ progress and outcomes, conducting regular check-ins and adjusting care plans as needed to ensure optimal outcomes.
• Facilitate communication and collaboration among members of the care team, including therapists, psychiatrists, and other healthcare providers.
• Educate clients and their families about mental health conditions, treatment options, and self-care strategies to promote recovery and resilience.
• Document all client interactions, assessments, and care coordination activities in accordance with HIPAA regulations and company policies.
• Learn and be proficient in our EMR system.
• Participate in ongoing training and professional development to enhance knowledge and skills in care coordination and mental health support.

Qualifications

• Certification as a Care Coordinator (e.g., CCM, CPCC, or similar) required.
• Minimum of 2 years of experience in care coordination, case management, or related field, preferably in a mental health or healthcare setting.
• Experience working with Substance Use Disorder preferred.
• Strong interpersonal and communication skills, with the ability to establish rapport and build trusting relationships with clients and their families.
• Knowledge of mental health conditions, treatment modalities, and community resources.
• Ability to work collaboratively as part of a multidisciplinary team and independently manage a caseload of clients.
• Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines in a fast-paced environment.
• Proficiency in electronic health record (EHR) systems and Microsoft Office suite.
• Commitment to maintaining confidentiality and protecting sensitive client information.

Other Requirements

• Must be available to work full-time.
• Must be able to clear a Level 2 background check.
