Are you a Speech Language Pathologist (SLP) with a desire to impact the lives of the older adult population? If so, our Rehab Team at Commonheart Home Health in Partnership with Symbria is seeking its next employee-owner team member.

NOTE: This is a Home Health role (NOT REMOTE) - seeking SLP to cover the West Bend, WI area - may be up to 30 mile radius but willing to consider smaller territory. Mileage reimbursement is offered per company policy. NO HOME HEALTH EXPERIENCE REQUIRED.

We can also consider Part Time for weekly regular scheduled hours between 20-29 or PRN Status for flexibility (no minimum weekly hours required).

Responsibilities

As a PRN Speech Language Pathologist (SLP), you will establish appropriate goals and creatively execute the treatment plans along with developing and implementing clinical and educational SLP programs. As a member of our interdisciplinary team, you will be required to maintain productivity standards and complete required documentation. More importantly, you will help your patients achieve critical outcomes while acting as a trusted partner to the community you serve.

Qualifications

• Be a graduate of an accredited school for Speech Language Pathologists.
• Hold a valid state license.
• Clinical Fellows welcome to apply!
• Home Health or other experience is great, but not required.
