Job Summary:

We are looking for a compassionate and experienced Registered Nurse to join our Home Health team.

As a Registered Nurse - Home Health,

you will play a vital role in delivering high-quality care to patients within the comfort of their homes. This position offers a fulfilling opportunity to positively impact the lives of individuals and families in need.

Duties:

- Conduct thorough assessments of patients' health conditions

- Develop personalized care plans tailored to each patient's needs

- Administer prescribed medications and treatments under physician guidance

- Monitor and accurately record vital signs such as blood pressure, heart rate, and temperature

- Provide patient education and support, involving their families in the care process

- Collaborate with a multidisciplinary healthcare team to ensure coordinated patient care

- Perform wound care procedures, including suturing when required

- Assist patients with transfers and mobility challenges

- Maintain detailed and timely documentation of all patient interactions Experience: - Possess a valid RN license in the state of practice

- Minimum of 2 years of experience in pediatric or home health nursing is preferred

- Proficient in conducting patient assessments and developing individualized care plans

- Skilled in administering injections and conducting wound care treatments

- Strong understanding of occupational health and safety protocols

- Ability to work autonomously while effectively prioritizing tasks

- Excellent communication skills and the ability to build rapport with patients and colleagues

Preferred Skills:
