About our team:A highly skilled and driven team united by a dedication to senior careExceptional leadership ensuring a safe, welcoming, and inclusive workplace culture A strong dedication to the growth and development of our team members.Must haves:High School Diploma or GED equivalent (preferred)Proof of current 2-Step TB testingFirst Aid and CPR CertifiedMust be bondableMeet or exceed minimum qualifications for the following:A criminal background check (National and required by state)Motor Vehicle Driving RecordProfessional and Personal Reference CheckMust be physically able to perform the functions of this positionHave reliable transportation, a valid driver's license, and proof of sufficient automobile insurance (when applicable)Satisfactorily complete SYNERGY HomeCare Orientation and online training modules as assignedHave the ability to learn and possess excellent communication skills both verbal and writtenHave the ability to learn and possess good organizational skillsEssential Duties and Responsibilities: Understand and implement a client's non-medical needs in accordance with the established Plan of CareProvides companionship for a client including, but not limited to:Talking and attentive ListeningProviding social and emotional supportPromoting mental alertness through client engagement and activities of client's interestPromotes a sense of safety and well-being (Notify the SYNERGY HomeCare office of safety concerns)Assists in meeting nutritional needs by providing meal planning, preparation, and grocery shopping as needed by the clientFollow SYNERGY HomeCare's Infection Control Policies (notify the SYNERGY HomeCare office if in need of appropriate PPE or supplies)Follow the eRSP telephony/mobile app/or other timekeeping methods instructions when clocking in and out for all scheduled shiftsMedication remindersObserve and report any/all changes in the client's mental, physical, or emotional condition or home situation to the immediate supervisor in a timely mannerEstablish and maintain effective communication and professional relationships with clients, family members, or other members of the client's support systemCompletes required documentation in a timely manner and submits to the SYNERGY HomeCare office according to policyUtilizes any/all equipment and supplies safely and properly
Maintains confidentiality regarding client information (HIPAA)Adhere to the SYNERGY HomeCare dress code policy as specified in the Employee HandbookReport to work on time and ready to work for the day as scheduled shifts requireAdhere to all SYNERGY
