SYNERGY HomeCare of the West Valley is a fast-growing, locally owned home care agency serving clients in Phoenix and the West Valley. Our office is a fun place to work, with a team of wonderful people working together to provide the best solutions for our clients and caregivers. We are searching for a dynamic, detail-oriented, and upbeat team player to join our office in Goodyear, AZ. This is a full-time position, 8 hours per day, 5 days per week. (Start time and days worked are negotiable.)

Job Summary:

The Care Coordinator reports directly to the Administrator and is responsible for guiding and coordinating all scheduling-related activities of the SYNERGY HomeCare business. This position includes leadership in areas of the business including maintaining positive relationships with caregiver and clients and organizing schedules to accommodate both clients and caregivers.

This position entails establishing a positive office culture where caregivers want to work, promoting office initiatives, and keeping good communication with caregivers. The Care Coordinator will be responsible for staffing all requested shifts and new client cases. Additionally, the Care Coordinator will be responsible for collaborating with the Administrator to enhance employee satisfaction and morale as well as revenue growth.

Qualifications:

  • Knowledge of scheduling and on-call procedures
  • Minimum of one (1) year experience in a business or home care office setting preferred
  • Proficient in computer applications, phone etiquette, email, and iPad
  • Must possess leadership skills, human relations abilities, customer satisfactions skills, and organizational skills.
  • Knowledge of scheduling software preferred.
  • Professional attitude, both in person and on the phone.
  • Mastery of English language and grammar required, both written and verbally. (Bilingual Spanish a plus)
  • Must be able to communicate effectively and tactfully.
  • Detail-oriented. Must be able to accurately enter data with excellent attention to detail.
  • Must be able to handle difficult situations with good judgment and possess personal integrity.
  • Background clearance required.

Duties and Responsibilities include:

  • Regular communication with caregivers.
  • Regular communication with clients and their families.
  • Schedule shifts and hours by matching caregiver qualifications and availability to client’s needs.
  • Accurately input client and employee data with excellent attention to detail.
  • Processes data necessary to initiate accurate payroll and billing processes.
  • Assist in interviewing caregivers, conducting background investigations and checking references for potential employees.
  • Works with Client Care Manager and Employee Care Manager to assist in resolution of caregiver issues.
  • Assist in supervising field staff and providing training and/or coaching when appropriate.
  • Develop a positive, friendly relationship with referral partners, clients, and caregivers.
  • Reliable and responsive, with the ability to convey a high degree of trust, empathy, and patience.
  • Communicates clearly and understands how to relate to clients, caregivers and referral partners such as case managers, social workers, etc.
  • Maintains a professional office environment while promoting a positive image for the company.
  • Available for After Hours Phone Rotation. This will be a rotating schedule within the office.
  • Provide support to other team members as needed.

Annual pay is $37,500 to $46,000 Depending on Experience
