Tahoe Coalition For The Homeless is Hiring a Case Manager Near South Lake Tahoe, CA

CASE MANAGER
Job Description | Updated February 27, 2023

Description: Tahoe Coalition for the Homeless (TCH) seeks a part-time (20 hours per week) Case Manager to provide skilled support to individuals experiencing homelessness, at-risk of experiencing homelessness, and who are previously homeless in their efforts to establish and maintain permanent housing.

Goal: To work as part of a multidisciplinary team with other client support staff to assist and support clients in establishing and meeting their individual housing goals.

Specific Duties:

1. Maintain a caseload of vulnerable individuals while also interacting with other clients on an as needed basis.

2. Implementing client-driven case plans with both short and long-term goals to meet client needs.

3. Partner with other program staff and community partners to develop and facilitate educational and life skills groups designed to enhance clients’ housing maintenance, explore and resolve self-care issues, and increase clients’ income and skill levels

4. Work with program participants to enhance and improve their activities of daily living (healthy cooking skills, navigating public transit, relationship building, and good neighbor skills).

5. Provide clients with connectivity support, including email, phone, and mail services.

6. Provide clients with transportation to medical or legal appointments.
7. Assist clients with job readiness and employment assistance services as needed.
8. Utilize trauma-informed, client-centered strategies to meet clients ‘where they are’.
9. Cultivate a working knowledge of local resources and build relationships with service partners in order to make effective referrals for clients.
10.Participate in regular staff meetings and work closely with team members to coordinate about client case plans, and provide support to each other as needed.
11. Maintain required records accurately, comprehensively, and in a timely manner, including data entry into the Homeless Management Information System (HMIS).
12.Engage clients in a trusting and supportive treatment partnership, providing emotional support while maintaining professional boundaries.
13.Maintain one’s own physical, mental and emotional well-being to maximize the healthy functioning of the program.

Requirements:

● Associates degree in social sciences, public policy or administration, drug and alcohol counseling, or related field.

● Minimum three years of related work experience, to include working with clients who are experiencing homelessness, mental illness, and substance use.

● Must maintain a valid Driver’s License and minimum automobile insurance required by the State of California.

● Ability to maintain clear personal and professional boundaries with clients.
● Ability to work effectively in constantly changing and sometimes demanding or stressful environments.
● Knowledge of local community resources.
● Effective organizational and time management skills.
● Excellent interpersonal skills including, conflict resolution, listening, and problem-solving skills
● Strong oral and written communication skills and intermediate knowledge of

Microsoft Office/Google Suite.
● Ability to remain alert and on duty throughout shifts.
● Ability to respond responsibly in emergency and crisis situations utilizing non-violent crisis intervention.
● Understanding and support of Housing First principles as well as Harm

Reduction Theory and practice.
● Experience using Motivational Interviewing techniques.
● Ability to effectively interact with clients, co-workers, and volunteers with diverse ethnic and cultural backgrounds and treat each individual with respect, dignity, and compassion.

● Physical ability to lift 25 pounds, sit, stand, bend, reach, and perform cleaning duties.

● The employee must be comfortable traveling in outdoor environmental conditions such as rain, snow, sleet, and warm temperatures typical of the South Lake
Tahoe Basin.

● Satisfactory completion of background and reference checks.
● Test negative for TB.
● Provide proof of vaccination for COVID-19.

Hours: Typically, the schedule is 9:00 AM - 5:30 PM, Monday - Friday. Schedule may be subject to change to accommodate programmatic needs. May require some on-call, evening, and weekend hours.

Rate: The hourly rate range for this position is $20-$25.

Class: The Case Manager position is non-exempt hourly, up to 20 hours per week.

Reporting: The Case Manager reports to the Program Manager.
