Duration: 8 weeks

Hours: 40hrs/week

Shift Timing: Day, 9AM - 5PM

License and certifications:

•Active New York State License as Registered Nurse

•BLS (AHA)

Responsibilities:

•Assess and evaluate members in MLTC and MAP lines of business, utilizing UAS information.

•Foster collaboration with PCPs, engaging in discussions regarding member care.

•Review assessment findings with PCPs to uncover any overlooked concerns.

•Identify member risk factors and categorize accordingly.

•Prior approve additional services based on assessments and evidence-based standards, referring complex cases to the Medical Director.

•Contribute to team care planning meetings.

•Address and resolve complaints promptly, assist in grievances, and support Quality Management Department requests.

Experience:

Two (2) to three (3) years clinical experience in certified home health agency (CHHA), Lombardi program and/or MLTC or MAP program.

Education:

Bachelor’s Degree required

BSN preferred.
