Facility: ProHealth Waukesha Memorial Hospital

Contract length: 13 weeks **

Start Date: 7/1 **

Guaranteed Hours Per Week: 36 **

Shift Requirements: 12 hour NOCs; every other weekend and holiday **

Local Pay:$70 an hour **

Weekly Gross 2,602.00

Weekly Taxable 1,440.00

Weekly Non-Taxable 1,162.00

Hourly Taxable up to 40 Hours 40.00

Hourly Non-Taxable 32.28

Overtime after 40 Hours 60

Minimum Experience: At least 3 years of ICU experience required. At least 1 travel contract completed preferred.

Required Certifications to be Attached upon Submittal: BLS, ACLS, NIHSS, WI RN
