Tandym Group is Hiring a Field Supervision Registered Nurse Near Queens, NY

A healthcare services company is seeking a skilled Field Registered Nurse (RN) Supervisor to join their team.
The Field Registered Nurse (RN) Supervisor makes initial and follow-up patient and home assessment visits to determine service needs, develop Plan of Care, instruct, and supervise the paraprofessional staff. The nurse sets up and updates patient's care records after every follow-up visit. This position requires traveling to different counties.
Qualifications:

  • Registered Professional nurse licensed in the state of New York.
  • At least two years' experience as a Registered or practical Nurse with one year field supervisory experience in community health.
  • Completion of a baccalaureate degree in nursing or an associate degree with at least 1-year supervisory experience.

Job Description:

  • Completes comprehensive assessments of patient and family to determine Home Care needs at initiation of services.
  • Develops and initiates the plan of care with patient's physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family in the planning process.
  • Instructs, supervises, and evaluates home health aide care provided as stipulated by law.
  • Performs follow-up visits as needed for recertification, staff supervision and change in condition.
  • Makes necessary revisions to patient's plan of care as patient status and needs change.
  • Provides health care instructions to the patient/family as appropriate per assessment and plan of care
  • Communicates with the physician regarding the patient's needs and reports and changes in the patient's condition; obtains/receives physician's orders as required.
  • Anticipates the needs of the patient, and recommends changes to Doctor and family as appropriate
  • Participate in clinical on-call coverage as requested and ensure active and timely responses.
  • Assists with any other job duties as assigned by DPS.
