A social services organization on Long Island is actively seeking a driven and dedicated professional to join their staff in Suffolk County as their new Intake Coordinator.

Responsibilities :

The Intake Coordinator will :

• Conduct screenings and intakes of prospective program participants
• Respond to program participants concerns, needs, and complaints promptly
• Obtain all necessary information and documentation to determine program eligibility
• Communicate effectively with the service team to rapidly assign cases and address issues from presenting veterans
• Integrate high priority Veterans into services where they may engage in comprehensive transition planning
• Act as advocate for program participants as appropriate; encourage and support self advocacy
• Interface with representatives from community agencies and partnering service providers
• Ensure that all information is collected and notes are recorded in Homeless Management Information System (HMIS)
• Ensure compliance with funding requirements and agency quality assurance standards
• Perform other duties, as needed

Qualifications :

• Bachelor’s Degree in Psychology, Social Work, and / or related Human Services field
• Microsoft Office proficient
• Solid problem solving and time management skills
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly orgnized

Desired Skills :

• Master's Degree
• Veteran or member of Veteran family
• Experience with Homeless families and / or Veteran population
• Experience with Electronic Database systems (such as HMIS)
