A health system in Tacoma, WA, is actively seeking a dedicated Nurse Practitioner to join their team. In this role, you will lead the medical aspects of team-based care for individuals with serious mental illness, conducting whole-person assessments while collaborating closely with community primary care and behavioral health providers.Responsibilities:

• Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health priorities.
• Support care coordination efforts.
• Lead care and disease management initiatives, identifying actionable care gaps.
• Communicate and coordinate with individuals' care providers, primarily at CMHCs, FQHCs, and hospitals.
• Provide direct patient care when unscheduled needs cannot be met by existing community providers.

• Washington State License

• Experience working with behavioral health patients.
• 2 years of experience as an NP/PA.
