A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff as a UAS RN. In this field role, the Assessment Nurse will be responsible for doing UAS assessments in the Bronx.

- Performing home visits to conduct functional, psychosocial and environmental safety assessments.
- Determining eligibility of enrollees.
- Establishing an individual plan of care that enables them to remain safely at home despite long-term care needs.
- Ensuring that DOH UAS information is complete and accurate
- Conducting follow-up assessments to ensure the members are receiving the proper care and services.
- Performing other duties, as needed.
- NYS Registered Nurse (RN) license
- Prior UAS, MLTC or Home Care experience
- Bachelor's Degree in Nursing
