A healthcare network in New York is currently seeking a licensed Nurse Practitioner (NP) to join their hospice team in the Bronx.

Responsibilities:

The Nurse Practitioner (NP) will:

• Coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
• contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
• Provide in-person and telehealth visits, as needed
• Provide support and guidance to patient's family members and caregivers, as necessary
• Other duties, as needed

Qualifications:

• Bachelor's and/or Master's Degree
• NYS Nurse Practitioner (NP) license

Desired Skills:

• Previous experience in hospice and palliative care
