Position:Intake Coordinator

Department: Referral Management

Reports to: Intake Supervisor

Position Description:

The Intake Coordinator is responsible for accurately entering referrals and verifying eligibility on each referral requests received. The Intake Coordinator maintains accurate records in compliance with applicable laws and regulations and the policies of tango.

Office Location:

• Office located at 7600 North 16th Street, Suite 140 Phoenix, AZ 85020 (Remote)

Responsibilities and Duties:

Responsibilities of the Intake Coordinator include, but are not limited to the following:

• Enter patient and referral information into tango software system.
• Verify insurance eligibility for each patient.
• Coordinate with the Qualification team as needed.
• Ensures service requests include all required documentation for verifying patient eligibility.
• Call patients and referral sources to obtain any additional information needed related to verifying eligibility.

Qualifications:

• Must be at least 18 years of age.
• High School Diploma or GED, some college
• High typing speed and accuracy.
• Has basic computer knowledge and experience including Microsoft Word, Excel & typing skills.
• Home care/healthcare background is a plus but not required.
• Must be flexible and willing to work weekends and Holidays.

• Is customer service oriented with good oral and written communication skills.
• Is detail oriented and displays good organizational skills.
• Capable of following an issue through to its conclusion

• Ability to maintain a high level of confidentiality and remain HIPAA compliant.
• Ability to establish highly productive and detailed organizational skills/habits.
• Possess strong, professional written and oral communication skills for in-person, telephonic, and electronic use.

• Thurs-Mon 830am-530pm AZ
• Fri-Tues 830am-530pm AZ
• Sat-Wed 830am-530pm AZ
