Summary:The Coordinator, Care Team is at the core of Tapestry 360 Health's team-based care model. The Care Team Coordinator (CTC) participates on a care team with a provider and one other CTC, working with them to help meet patient needs. The role is patient-facing and responsibilities include setting visit agendas, gathering patient information, and performing vital signs. Other tasks include documentation support, facilitating communication between the patient and the rest of the care team, and ensuring patient understanding of their care plan. The CTC is critical in building trusting, supportive, and healing relationships with patients. The CTC role is an ideal position for someone who wants to work (and learn!) in a team environment and is passionate about helping to transform healthcare. Visit-based skills and experience will be gained through the onboarding process and continuous coaching.

As an integral Care Team member, Tap360 CTCs provide clinical services to patients from the waiting area to the exam room, document care in the EMR, and assist providers with routine care, lab procedures and administrative tasks.

Major Responsibilities:

• Promptly and effectively support patient care delivery
• Greet, vital and screen patients in preparation for provider visit
• Recordkeeping
• Assist front desk staff and nurses, when required 

Minimum Required Qualifications:

• Successful completion of a Medical Assistant (MA), Practical Nursing, Paramedic or Emergency Medical Technician program or
• At least 1 year of previous MA experience in a clinical setting
• Current Basic Life Support/CPR certification
• Excellent customer service and nonverbal active listening skills 

Desired Qualifications:

• Bilingual
• Experience using EMR systems (e.g., athenaOne, Centricity, Epic, Praxis, Meditech, Cerner, etc.)
• Phlebotomy skills
