Tarzana Treatment Center is Hiring a Child & Family Specialist II Near Los Angeles, CA
Position SummaryResponsible for working directly with children/youth and their family in their respective environments to help them achieve a permanent placement for the youth/child. This position housing within the DMH Wraparound program requires 24-hr coverage, seven (7) days a week, including weekends. The program involves the coordination of care and intervention for high-risk children/youth being reunited with their families. The overall program goal is to assist the family attain self-sufficiency. The Child and Family Specialist reports to the Program Supervisor for administrative activities and the Clinical Supervisor for clinical issues Salary Range$24.75 - $28.50 per hour DOE HOURS Hours vary depending upon availability of the families but requires on-call 24-hour coverage and availability 7 days per week. CATEGORIES OF DUTIES
Typically has a caseload of up to ten (10) clients with the potential to increase to up to fifteen (15)
clients at the discretion of the program.
Completes needs assessment designed to identify patient needs unique case.
Responsible for completion of a resource assessment for all children/youth to identify available
resources and programs designed to meet those needs that already exist in the patient’s community.
Conduct regular reviews in order for the plans to be fluid and evolve as the patient responds to and
is impacted by the services they are linked with.
Works closely with the Facilitator to ensure coordination of services available in the community.
Attends all required Wraparound Program meetings.
Ensuring there is continuous input and feedback from the family and services providers.
Cooperates and communicates fully with other staff members regarding patient behavior, progress,
activities, etc.
Conducts self in a professional manner in areas of dress, language, attitude, and behavior.
Exhibits full knowledge and support of the philosophy and objectives of the program and agency.
Fully understands and maintains policies regarding professional ethics, including appropriate
boundaries and patient confidentiality.
Proper documentation in the electronic health record of services rendered.
Participation in medical record reviews.
Knowledge of current legal standards pertaining to professional practice, reporting requirements
(i.e., suspected abuse; Tarasoff), professional boundaries, and related issues.
Assist in conducting performance evaluations of staff, as required.
Additional functions as specifically requested by the Operations Supervisor, Clinical Supervisor,
Program Director of Youth Services, and Administration. Education/Experience/Qualifications
BA/BS in psychology, social work, or other health related field and a minimum of one (1) year
experience working with at-risk youth. Preference given to individuals with a Master’s level education and relevant work experience.
Medi-Cal Peer Support Specialist Certification (PSCC)
Valid California driver’s license, vehicle for transportation, and applicable car insurance.
