Position Summary

The QI Coordinator is responsible for the overall direction, coordination, and implementation of the QI Program for the Clinic. This will include the collection, management and analysis of quality data and the preparation of quality reports. Practical knowledge of tools and techniques of Continuous Quality Improvement, including analysis and interpretation of data using computer-based disease registries or similar data collection systems is essential. Must be familiar with UDS and HEDIS reporting as well as Joint Commission accreditation. The QI Coordinator will participate in and assist in preparation for all clinical audits and surveys. The QI Coordinator will also be involved in the implementation, maintenance and regular development of the Primary Care EHR.

40 per week, Monday-Friday

Categories of Duties

* Assists in the development, monitoring, and presentation of internal quality measures and initiatives

* Coordinating tracking and reporting of clinical outcomes, and follow-up of corrective action plans

* Responsible for developing methods for data collection and extracts data as required

* Assists with coordination of weekly QI Meetings

* Prepares QI reports and statistics

* Participates in staff and management meetings as they relate to QI activities

* Provides in-service training to non-provider staff in quality improvement

* Reviews QI tools and surveys and provides technical assistance to staff

* Oversees annual UDS, HRSA and OSHPD reports

* Supports the achievement of population health and clinical quality goals (e.g. Diabetes population management)

* Acts as point person on implementation and development of Primary Care EHR, including:

* Analyzes and documents current and proposed clinical workflows, as they pertain to the implementation of electronic practice management and electronic medical records software

* Redesigns existing workflows to leverage the EHR application and documents, reviews, and trains future state workflows

* Directly and remotely supports the physicians and physician office staff in the use of the software

* Attends conferences and training sessions as requested

* Performs other related duties as assigned by CEO, Director of Information Technology or QA Supervisor.
