A fast growing home care agency is looking to hire Full Time/Part Time Registered Nurses to join their dynamic team with lots of growth potential!

You will play a vital role in providing high quality and compassionate health care to the clients and families of the agency.

Responsibilities:

- Conduct initial assessments of new clients to determine their healthcare needs and develop

care plans based on their specific needs.

- Perform regular home visits to assess client progress and make adjustments when necessary.

- Provide education and emotional support to clients and their families.

- Maintain accurate and up-to-date patient records and documentation.

- Participate in team meetings and contribute to the development of care strategies.

Qualifications:

- Current NYS RN license

- One year of nursing experience, preferably in home care

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassion and dedication to providing patient-centered care

- Ability to multi task

- Reliable transportation and valid drivers license
