Location: Kent, Ohio

Funding: Homecare Waiver (ODM) and IO (DODD)

Services: PDN or DODD IO waiver nursing

Day shifts: Monday, Tuesday & Wednesday and, every other Saturday & Sunday (8am-4pm)

Night Shifts: Monday & Wednesday (10pm to 8:00am) and, Every other Tuesday, Thursday & Saturday (10pm to 8:00am).

Note: Shift start & stop times may be modified to meet nurses schedules.

Profile: 61yr old, quadriplegic (diving accident at age 19; C5-6 SCI à paralysis from chest down, Weight about 106 lbs. Retired from full-time position at KSU. Lives in home w/husband near KSU. Pleasant, motivated client. Uses motorized wheelchair, hospital bed & ceiling Hoyer lift for easy dressing and transfers. Has trach.(capped by day & uses O2 overnight).. Has indwelling Foley catheter. Requires passive and resistive range of motion, BP management, dressing & assistance w/ADL hygiene and, placement of adaptive cuffs and equipment to maintain and improve independence.
