Teleios Collaborative Network is Hiring a Registered Nurse, CAPABLE Program, PRN Near Savannah, GA
PRN Registered Nurse Schedule: 10-15 hours/week, Flexible Summary: The CAPABLE Registered Nurse (RN) collaborates with older adults, delivering nursing components of the program after completing required training from Johns Hopkins University School of Nursing. CAPABLE is a home-based program for older adults that aims to address functional opportunities and promote independence. The RN will visit participants' homes, assess their goals, and suggest strategies. The CAPABLE program consists of up to four RN home visits over 4-5 months. Clinicians meet with participants in their home unless otherwise determined. The RN uses training and motivational interviewing techniques to conduct a comprehensive interview to identify and prioritize issues that matter to the participant. These goals become the foundation for brainstorming and action plan development and implementation. The RN documents the participant's own goals in up to 3 Action Plans. The RN interacts and communicates with the CAPABLE Occupational Therapist (OT) and other team members as needed to ensure close collaboration throughout and after the program. Qualifications:
A state-issued nursing license is required for this role. Ability to work from a collaborative, client-directed model.
Willingness and ability to implement a protocol and balance with clinical judgment. Bachelor's degree in nursing
Experience in home care preferred.
Physical Demands:
The position demands moderate visual concentration for effective patient charting/documentation.
This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions.
The RN requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.
Work Environment:
A part-time position works a 16-29 hour work week with occasional requests to work additional hours as needed.
Due to patient census and acuity, this role may occasionally need to stay on-site at Hospice House during meal breaks.
The RN position may require them to provide care in high-risk crime areas determined by local authorities.
This position encounters stressful situations with patients and their families regarding end of life care.
The RN must be able to accurately, timely, and concisely document the patient/family encounter using a computer.
The RN is expected to conduct home visits by utilizing their own personal vehicle.
Each RN is assigned to a specific department/region, but may occasionally need to serve patients outside of their designated area.
