Job Title:  On-Call Nurse| Home Care & Hospice

Job Type:  Per-Diem

Location:  Somerset & Middlesex County | Somerville, NJ

Rates:

• Flat rate & Additional rates per visit
• $3,000 sign-on bonus

Education/Experience:

• Current NJ RN License
• Valid Driver’s License required
• One year exp. in home health or hospice preferred

Job Summary : On-call nurse triages after-hour calls received from the answering service. Communicates with patient, assesses the situation, provides instruction, and/or determines if a visit is required.

Job Description:

• Provides safe on-call triaging for both Home Care and Hospice under medical direction and in accordance with recognized standards of nursing practice and established policies of the agency.
• Responds to calls forwarded from answering service, places call to patient.
• Assesses the needs of the client, develops a plan of action, and determines if phone call is sufficient, need for a home visit, physician notification, and/or 911, emergency room.
• If a home visit is required, assesses the situation, determines plan of care, contacts the physician as necessary, observes the response, and documents results.
