POSITION: Housing Case Manager
PAY RANGE: $20-22
PROGRAM SUMMARY:
The Barrett Foundation Housing Case Managers work with displaced individuals and families to secure safe and habitable housing. Once a household has secured housing, the Housing Case Managers works with each household to complete a monthly supportive service plan. Case management services are comprehensive in nature to ensure a more effective service delivery but are tailored to the varying needs of the program participant(s).
RESPONSIBILITIES:
- Conduct program participant screenings and intakes
- Support Housing Placement including housing search, screening potential landlords, housing application process, ensuring appropriate inspections are conducted and other move-in activities
- Negotiate and obtain leases according to grant guidelines and agency procedure
- Assist in building landlord and tenant relationship
- Create a monthly supportive services plan to address barriers that may affect housing stability
- Work with each program participant to develop an exit plan to permanent housing
- Monitor program participants’ progress by completing interim reports when household changes occur
- Meet with program participants via home visits, office visits, telephone, and video conference; in- home visit required at minimum every 30 days
- Ensure that clients have adequate access and information about healthcare and other supportive services
- Provide crisis intervention as necessary
- Maintain client files and ensure compliance with funder requirements
- Ensure confidentiality of program participants and of their records
- Attend staff meetings, trainings, and peer reviews as necessary
- Attend supervision meeting with direct supervisor
- Advise Lead Case Manager of adverse changes in program participant progress or needs for support
- Conduct program discharges
- Knowledge of Federal, State and Local rules and regulations governing subsidized housing programs
- Awareness of emergency procedures
- Perform other duties as assigned

OTHER REQUIREMENTS:
- Valid Driver’s License (required)
- First Aid & CPR (preferred)
- Bilingual (preferred)
EDUCATION & EXPERIENCE:
- Bachelor’s degree in counseling, social work, social sciences, or related studies preferred with 2 years of housing case management/case management experience preferred
- Associates degree in counseling, social work, social sciences, or related studies preferred with 2-3 years of housing case management/case management experience preferred
- High School diploma or GED with minimum 4 years of housing case management/case management experience or commensurate experience with transferable skills
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
