Actively seeking candidates to collaborate with Quality Medical Director and Quality team to improve performance in various clinical metrics. Responsibilities include working with patients via telephone, virtual, and in person meetings to education on the need for clinical interventions, engage in their health plan benefits, promote medication adherence, assess and evaluate needs using various tools and resources, such as EPIC and payer date as well as collaborating with providers and payers as needed to support plan of care.

Previous experience in quality management is preferred.
