Description

The Health Homes Care Coordination Supervisor and Quality Specialist works towards achieving the six core care management functions of the designated health home program. Works with and provides direct supervision to care coordinators to ensure the efficient and effective delivery of care management services. Provides oversight to their designated supervisees to ensure that all members meet the goals of their comprehensive patient/family focus plans of care. Assist with ongoing training and the identification of resources and referrals to meet the needs of the clients. Will assist the Director in providing professional development opportunities for all staff.

Essential Position Functions:

• The Care Coordination Supervisor will supervise and oversee the daily activities of the Care Coordination team.
• The Care Coordination Supervisor will be expected to utilize all electronic databases as required by Health Homes requirements.
• Participate in group and individual supervision as indicated by the Program Director to ensure that the services for Health Home consumers are being rendered appropriately
• Initiate ongoing team meetings with Care Coordinators to implement and review goals, plans of care, comprehensive assessment, HARP, core services, and timely documentation while supporting the training and team building for all staff.
• The Care Coordination Supervisor is tasked with servicing caseloads of specific individuals needing additional expertise and stabilization including AOT consumers who will be stepped down to Care Coordination.
• Address issues and concerns, which may arise for the client throughout they enrollment period within the Health Home program.
• Utilize a client-centered, goal-oriented, and data-driven approach to supervision.
• Provide ongoing monitoring to ensure that client charts and documentation are in compliance with City, State, and Federal guidelines.
• Provide direct one-to-one and group clinical supervision for the Care Coordinators.
• Provide support around case conferencing and coordinate incident reporting procedures for the program.
• Complete all weekly and monthly reports as required.
• Undertake any additional tasks and initiatives as assigned by the Program Director and/or Senior Director.

Qualifications

Required Knowledge, Skills, and Abilities:

• Master's Degree Required
• 2 years of supervisory experience.
• Must have excellent computer skills, and knowledge of AWARDS 
• Must have good personnel management skills to work in a human services/rehabilitation environment.
• Must have excellent verbal and written communication skills, professional presentation, and excellent interpersonal/customer service skills.
• Bilingual is highly desirable.
