Job Details Job Location New York, NY Position Type Full-Time Education Level High School Salary Range $38,000.00 - $40,000.00 Salary/year Job Shift Monday - Friday 9 AM - 5 PM Description The Intensive Mobile Treatment Team is a mobile team that aims to improve program participant's quality of life through engagement in treatment, rehabilitation, housing, and support services that promote wellness, recovery, community integration, as well as community safety. The IMT team provides these services and/or, where appropriate, links consumers to these services. The Intensive Mobile Treatment Team is guided by the principles of access, engagement, continuity of care, and awareness of risk. Services include but are not limited to assessment and treatment planning, crisis, intervention, medication management, medication-assisted treatment, integrated mental health, substance use treatment, individual and group therapies, psycho-education, psychosocial and vocational rehabilitation, assistance with medical concerns and medication, linkage to follow -up assistance to primary medical care including on-going assistance with activities of daily living, entitlements, housing procurement and support, linkage and coordination with the criminal justice system, shelter system, and assistance in using family and community services. Position Overview : The Certified Peer Specialist is a key member of the Intensive Mobile Treatment Team (IMT). They use their lived experience to work with adults with highly unstable housing situations, unclear diagnoses, and/or interaction with the criminal-legal system who have often been unsuccessfully served by the existing service system. The Peer engages participants in peer-based services to identify and overcome barriers to community integration, utilizing a peer relationship to foster connection, trust, understanding, and validation with community providers and support. Our clients have histories of trauma, unstable housing &/or homelessness, incarceration, and hospitalizations and are primarily people of color who are victims of systemic racism. The work is conducted in the field, where our clients live and spend their time. Hours are typically 9 am-5 pm, Monday through Friday, with flexibility to meet the needs of the clients outside of those hours. 24/7 crisis intervention services are also expected and will be scheduled on a rotating basis within the team. Qualifications Required Knowledge, Skills, and Abilities: High school or equivalent (Preferred) Building relationships with program recipients and their networks of support to support the person's recovery. Assist individuals with navigating the service systems, including behavioral and medical health, criminal-legal, shelter system, entitlements, and transportation. Practice according to the principles of recovery-oriented and trauma-informed service delivery. Complete all required documentation accurately and timely. Participate in team meetings, including High-Risk meetings. Conduct individual sessions with program participants addressing issues that have served as barriers to independence. Willingness to share personal, practical experience, knowledge, and first-hand insight to benefit program participants. Currently certified as a Peer Specialist or Certified Recovery Peer Advocate or certified within a year of hire. The Bridge Inc. is an Affirmative Action / Equal Opportunity Employer
