The Children's Center, Inc | Careers is Hiring a Runaway and Homeless Youth Case Manager Near Victoria, TX

Position: Case Manager

Reports to: Senior Case Manager

Principal Purpose of Job:

This position involves working with Runaway and Homeless Youth who are victims of abuse, neglect, homelessness, and human trafficking, and/or who are high-risk for victimization. This position requires the ability to serve in an on-call capacity and respond to emergencies. May include work after hours, weekends, and holidays. Out of town and overnight travel may be requested.

Must be able to provide critical information to support other human services personnel, i.e. police, EMS, and other personnel responding to the crisis, and demonstrate the ability to document information in a clear, concise, and professional standard.

Trainings:

Completion of initial training requirements is mandatory for continued employment. Training must be completed annually to comply with licensing and accreditation standards, as well as TCCI policy. This position may require additional training to meet specific job requirements or enhance professional growth.

Duties include:

  • Conduct thorough assessments to identify clients' needs. May include interviewing clients, reviewing their records, and collaborating with other professionals.
  • Develop individualized service plans based on clients' needs and goals. This includes identifying appropriate interventions, resources, and support services to address clients' challenges.
  • Connect clients with relevant community resources, such as housing assistance, healthcare services, etc.
  • Advocate on behalf of clients to ensure they receive access to services and support systems. This may involve liaising with government agencies, healthcare providers, schools, and other organizations.
  • Coordinate services and support across multiple agencies and providers to ensure continuity of care.
  • Provide immediate support and intervention during crisis situations.
  • Maintain accurate and up-to-date records of client assessments, service plans, progress notes, and outcomes.
  • Must maintain client confidentiality.
  • Educate clients about available resources, rights, and responsibilities.
  • Receive crisis calls, make evaluations and referrals as necessary.
  • Regularly monitor clients' progress towards their goals and adjust service plans as needed.
  • Conduct outreach activities to raise awareness about available services. Build partnerships with community organizations.
  • Engage in ongoing training and professional development activities to enhance knowledge and skills in case management and trauma-informed care.

Position Qualifications:

  • Bilingual (English/Spanish)
  • Education:
    • Bachelor's Degree or any relevant experience in case management
  • Experience:
    • Prefer one year experience working with homeless and at-risk youth.
    • Proficient in Microsoft Office Suite
    • Strong interpersonal skills
    • Ability to build rapport, establish trust, and communicate effectively with clients, families, colleagues, and community partners.
    • Flexible and adaptable to work effectively in a fast-paced and dynamic environment.
