Job SummaryThe case management program support staff communicates with primary CM staff, transportation department and management to coordinate and transport clients to medical and court appearances. CM support staff will attend court proceedings with client, provide necessary documents of support i.e. progress reports and have knowledge of the case they are representing. The cm program support staff will assist in managing client cases by transporting clients to medical appointments and provide all necessary documentation requested by medical provider. CM program support will work closely with transportation department and clients to ensure court and medical appts are attended. The Case Management Program Staff is an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties:
CM Program Support Staff will work directly with Primary CM, transportation department and management to coordinate and transport clients to court and medical appointments.
CM Program Support Staff will fully understand the transportation scheduling process and utilize it to manage daily schedule and ensure clients are attending court and medical appointments.
Effectively communicate with management, primary case managers and transportation.
Maintain certification and or license while receiving weekly supervision to meet licensing board requirements.
Secondary FunctionsSecondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc.
Performs other duties as assigned by the Program Coordinator/Director.
COMPETENCIES
Demonstrates Knowledge in Substance use disorders, community psychiatric supportive treatment, ASAM LOC, medical necessity.
Demonstrates understanding of CM duties and performance including understanding the continuum of care at TCC, and confidentiality of client information.
Ability in the area of effective communication, interpersonal skills, written communication
Ability to complete all documentation in accordance with agency policy.
Effective time management/organizational skills.
Minimum Qualifications, Including Training And Experience
High School Diploma or equivalent, Behavioral Science Degree preferred.
CDCA or QMHS
Knowledge in Substance use disorders, community psychiatric supportive treatment, ASAM Level of Care and medical necessity.
12-Step Recovery knowledge
Valid Ohio Drivers License and ability to maintain insurability preferred.
