• Provide clinical/outpatient services for high-risk older adults in subacute and long-term skilled nursing facilities
• The physician will join a primary care team consisting of physicians and advanced practice providers who specialize in this care.
• Travel will be required throughout the Southeast Minnesota region, and can include shared time in Rochester.
