Night Shift Nurse Care Manager (RN)
We just opened a beautiful state of the art, five-story, 90-bed Health Center in February 2023! Join our Small House team and enjoy providing individualized care in a homelike setting with low staff to resident ratios! The Forest at Duke is a not-for-profit Continuing Care Retirement Community, and our The Health Center is a combination facility which offers Memory Care, Assisted Living, and Skilled Nursing.
Our new Health Center follows the Small House Model of Care and features 2 “households” of 10 residents per floor to make up a “neighborhood” of 20 residents.
The Small House Model of Care is a person-centered approach where residents have a significant say in their daily lives, their care, and their living environment. Residents’ care is resident directed, where the rhythm of each individual’s life is dictated by personal desires.
The Nurse Care Manager directs and provides person-centered, rehabilitative, holistic care and chronic disease management to residents. The Nurse provides leadership in all disciplines on their assigned neighborhood (floor) and ensures that care is provided in accordance with the care plan. The nurse has oversight and supervisory responsibility for day-to-day nursing care and programs and services provided by the neighborhood Care Partners in accordance with current federal, state, and local standards governing the facility.Key Responsibilities:
