You will direct the day-to-day operations by helping other nurses, caregivers, and NACs to do their best. Job DescriptionThe Hearthstone is seeking a successful and motivated Community Health Nurse to oversee and lead the Assisted Living clinical team and provide the coordination of care for our residents within the Assisted Living and Memory Care areas. Your success in this role will be achieved through excellent training, oversight, and a compassionate heart that is dedicated to providing the highest degree of quality for our resident's care. Some of the daily responsibilities of this role include:
- Supervising and coordinating care for residents in assisted living.
- Hiring, training, and developing the clinical staff, and ensuring staffing levels for all assisted living/memory care personnel are at levels required to meet the needs of residents served.
- Ensuring that a service agreement and assessment are established for each resident and that this plan is reviewed at least annually.
- Coordinating Risk Management processes through the Chief Healthcare Officer.
- Coordinating nursing services with other resident care services such as Physician, Physical and/or Occupational Therapy, Dietary, MSW, etc.
- Administering medication and treatments to residents as needed.
- Formulating and maintaining policies according to State regulations, Assisted Living standards of practice and the Hearthstone Mission statement.
- Ensuring trainings, orientation and in-service education is provided to staff in accordance with current Assisted Living Regulations.
- Assessing physical, spiritual, and psychosocial needs of residents and coordinating with consultants as needed.
- Providing emergency care.
- Maintaining ongoing communication with all departmental team members.
- Other duties as assigned by the Chief Healthcare Officer.
