Position Overview:

The Home Doctor is seeking a compassionate and dedicated Nurse Practitioner to join its team. As a Nurse Practitioner specializing in primary care, you will play a pivotal role in delivering comprehensive healthcare services to our diverse patient population across various living settings.

Responsibilities:

• Provide direct patient care, including conducting comprehensive assessments, diagnosing health conditions, prescribing medications, and developing personalized treatment plans.

• Collaborate closely with the interdisciplinary team to ensure coordinated and holistic care delivery.

• Conduct regular patient visits within their living environments, emphasizing continuity of care and personalized attention.

• Educate patients and their families on health maintenance, disease prevention, and treatment options.

• Maintain accurate and updated patient medical records using electronic health records (EHR) systems.

• Adhere to clinical protocols, evidence-based practices, and regulatory standards while delivering care.

• Demonstrate sensitivity, empathy, and cultural competence in addressing patients’ diverse needs and backgrounds.

Qualifications:

• Master’s degree in Nursing (MSN) from an accredited Nurse Practitioner program.

• Current and unrestricted Nurse Practitioner license in [state/region].

• Certification as a Nurse Practitioner specializing in adult-gerontology or family practice.

• Prior experience in primary care, preferably in a mobile or home-based healthcare setting preferred but not necessary.

• Strong clinical assessment and diagnostic skills.
