Interim HealthCare is seeking a dedicated and compassionate Private Duty LPN to join our team for a patient in Milton, New Hampshire. The Private Duty LPN will provide one-on-one care to a patient in his home, assisting with daily activities and providing professional medical care.
Responsibilities: Provide skilled nursing care to the patient, including medication administration, general care, and patient monitoring.Communicate with the patient's healthcare team, including the physician, nurse practitioner, and case manager as needed.Provide emotional support and comfort to the patient.
