Job description
POSITION SUMMARY: The Mobile Treatment Nurse directs nursing care and functions as part of a team providing services to patients in the community or in the office setting. This is an experienced psychiatric nursing role requiring skills for independent practice.
DUTIES AND RESPONSIBILITIES:
1. Coordinates the care of selected caseload of patients.
2. Participates in development of individualized treatment plans for assigned cases.
3. Works with professionals of other disciplines to plan and implement psychiatric nursing services.
4. Assesses health status of patients and identifies physical and psychiatric needs of the patient.
5. Records and reports health status of patients, treatments administered and other appropriate information on patient record, to patient’s doctor and other professionals serving the patient.
6. Will manage and maintain medical records of assigned Mobile Treatment cases while participating on Medical Record Review Team.
7. Observes the behavior and physical condition of patients to assess their mental health status.
8. Monitors health status of assigned patients and initiates patient referrals for identified health problems.
9. Provides crisis intervention, first aid, and general nursing procedures as needed.
10. Provide experienced level of services that may include service linkage, assessment and planning of services, patient advocacy, monitoring of service delivery, patient outreach, and documentation.
11. Participate in 24-hour emergency coverage rotation and provide crisis intervention services.
12. Functions as part of a continuous treatment team to serve and meet the needs of patients.
13. Provides liaison services for patients with various community agencies.
14. Document all clinical activities in accordance with agency requirements.
15. May provide medication education and consultation to other mental health professionals as needed..
16. Development and implementation of health assessments.
17. Plans health related groups and individual plans.
18. Participates in ongoing clinical supervision according to agency policy.
19. Performs other duties as necessary.
MINIMUM QUALIFICATIONS:
--- Current Maryland R.N. license.
--- Certified as a generalist psychiatric/mental health nurse by ANCC possession of Bachelor of Science Nursing (BSN) from an accredited program.
