The Health Equity Program Coordinator will plan, design, implement and evaluate health equity efforts to achieve compliance with regulatory requirements, as well as organizational patient experience, quality, and safety outcomes, for Nebraska Medicine patients. Collaborate with key stakeholders and organizational leaders to inform them regarding best practices related to disparity reductions, including the provision of health equity and social determinant of health resources for Nebraska Medicine patients. Summarize, analyze, and interpret data related to potential risks for inequitable quality, patient experience and safety outcomes. Lead, facilitate and manage cross functional improvement teams aimed at addressing identified opportunities. Align health equity efforts with Nebraska Medicine diversity and inclusion values and efforts. Serve as the expert for national quality payment programs and innovative payment models aimed at improving health equity within the health system. Collaborate with the analytics team to ensure Nebraska Medicine collects and meaningfully uses patient data such as race, ethnicity, language, and social drivers of health data to identify disparities. Coordinate and collaborate with Nebraska Medicine leaders, colleagues, physicians, local and state government, community-based organizations, and other entities to impact health disparities at a population level. Evaluate and track potential health disparities within the care of patients at Nebraska Medicine and advocate by prioritizing short, intermediate, and long-term goals. Partner with community organizations to help design and implement programs to help reduce disparities, that can be initiated during patient care interactions. Required Qualifications:• Bachelor’s degree in public health, clinical healthcare or social sciences required.• Minimum of three years of community relations or quality improvement experience required.• Clinical experience in quality improvement, outcomes monitoring, and/or process development in the healthcare arena required.• Exceptional communication, interpersonal and relationship building skills required, including experience discussing complex and potentially controversial topics.• Strong presentation skills required.• Demonstrated excellent organizational and communication skills (written and verbal), leadership, and facilitation abilities required.• Strong attention to detail, especially related to data management required. • Proficient in Microsoft Office Applications Required (e.g., PowerPoint, Excel, Access, Visio, and Word)• Ability to work effectively in teams. Demonstrated knowledge of team building, both as a participant of teams and in a leadership role required.• Understanding of basic principles related to promoting a culture of inclusion required.• Demonstrated knowledge in data collection, analysis and interpretation required, including exceptional skills with Microsoft excel.• Demonstrated critical thinking/analytical skills as well as strong problem-solving skills required. • Demonstrated strong interpersonal skills and capabilities, along with the ability to work cross-functionally with other leaders required. • Currently licensed as a Registered Nurse in the state of Nebraska or current compact state license or appropriate allied health license in the state of Nebraska only as required in accordance with appropriate degree required.Preferred Qualifications:• Master’s Degree preferred in public health or healthcare related field.• Demonstrated knowledge and application of DMAIC and/or LEAN principles preferred.• Healthcare Quality Certified (e.g., CPHQ, CHQ, other) preferred.• Experience with community health improvement efforts and community outreach efforts preferred.• Epic experience preferred.• Prior experience collaborating directly with physicians preferred.• Experience with health care related accreditation and regulatory agency standards preferred.• Experience with payer contracts, quality payment programs and regulatory requirements preferred.Nebraska Medicine is an Equal Opportunity Employer.
