Job Title: RN Case Manager Reports to: Director of Correctional Release Center Classification: Part-time, exempt MissionThe Next Door provides a continuum of evidence-based substance abuse and mental health services for women in environment of faith and healing to restore hope and a lifetime of recovery. SummaryRN case manager (CM) is responsible for planning, implementing, and coordinating health care resources/services to meet the physical and mental health needs of women at TND Correctional Release Center, preparing them for successful reentry into the community. Primary Responsibilities And Duties Include The Following
Initial admission assessment and medication reconciliation
Manage resident medication processes including organization, storage, receipt, and prescriptions
Provide oversight of the daily dispensing of medication.
Coordinate medication prescriptions and refills, communicate with medical providers, TND residents, and TND staff.
Collaborate and connect residents with community health care providers and pharmacies.
Participate in weekly treatment team meetings.
Ensure timely and professional documentation per TND guidelines.
Provide nursing triage to residents with medical or mental health concerns.
Administer basic first aid to residents if indicated.
Provide health education to residents.
Provide consultation to TND staff regarding resident’s health conditions and/or medications.
Additional responsibilities as assigned.
Education And Experience
Bachelor’s of Science or Associate’s degree in Registered Nursing
Minimum of 1 year experience as a Registered Nurse
Knowledge of psychiatric disorders and alcohol and substance abuse
Knowledge of community resources and health care service delivery across a continuum of care
