Title: Case Manager
Program: Access Center
Department: Access, Interim Housing, Mental Health & Sojourn domestic violence services
Status: Full time, Non-Exempt
Schedule: Monday – Friday 8:30AM to 5:00PM and 1st and 2nd Wednesdays of each month 8am-4:30pm
Essential Duties & Responsibilities
Under the supervision of the Program Manager, the Case Manger is responsible for:
1. Work with a diverse and marginalized client caseload experiencing many barriers including homelessness, complex trauma, medical, mental health and substance use needs
2. Ensure that case management services to clients are reflective of the “Whatever It Takes” model
3. Develop effective, trusting relationships with clients using a client-centered approach using best practices that include Client-centered, Motivational Interviewing, Harm Reduction, Non-violent Crisis Intervention, Trauma-Informed Care and Housing First
4. Perform a comprehensive needs assessment (screening and intake) and develop a housing stability plan that is based on client’s needs, issues and goals
5. Provide information, direct referrals and linkage, and advocacy to support clients in achieving their goals
6. Assist Clients in applying for and obtaining all benefits to which they are entitled.
7. Coordinate appointments, transportation, and follow-up services for clients to improve access to primary health care, mental health care, substance use services, and other local resources in order to build sustainable community connections and reduce the need for emergency health care services
8. Support Clients by teaching and modeling life skills involved such as budgeting, housekeeping, meal preparation, appointment scheduling, health maintenance, and socialization within communities where they become housed
9. Identify appropriate emergency and permanent housing options for clients
10. Directly transport clients in company van to various agencies and healthcare providers in the greater Los Angeles area to decrease barriers to access and increase community reintegration
11. Maintain accurate documentation of client information and records (intake, referrals, progress notes, service delivery, etc.) (manually and via computerized database system)
12. Co-facilitates Case Management orientation and information sessions
13. Participate in all case file, staff meetings, supervision, team meetings, training, and retreats
14. Participate in on-going self-evaluation, program evaluation, and development/modification
15. Provide Crisis intervention using Non-violent Crisis Intervention techniques
16. Communicate effectively and promptly with management team, peers, collaborators and clients
17. Other duties as assigned
Qualifications
1. Bachelor’s degree preferred; or a minimum two years’ experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience.
2. Demonstrated knowledge of issues faced by population served
3. Knowledge of social services agencies, community & housing resources.
4. Able to work in a high tolerance Harm Reduction model with members who have multiple barriers.
6. Ability to self-motivate, learn quickly, work effectively with a wide range of constituencies and be flexible in a fast-paced environment.
7. Ability to handle difficult situations and communicate effectively
8. Commitment to improving the community through empowerment and dignity
9. Computer literate; able to use computerized database system for program participant file management and proficient of Microsoft Word, Outlook, and Excel.
10. Current, valid California Driver’s License with an acceptable driving record, and reliable vehicle.
11. Comfortable, able and willing to drive/transport program participants in an agency van.
12. Bilingual preferred, but not required
13. Able to provide cleared TB test & pass a criminal background check.
Work Environment
1. Combination of field and office environment
2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
3. On occasion walk or drive to different local sites
4. Will necessitate working in busy and loud environments
5. Will be exposed to elements like cold, heat, dust, noise and odor
6. May need to bend, stoop, twist and sit throughout the day
