The Plus Group - Modesto is the leading Medical Staffing Firm for all of Central Valley areas. We are currently hiring for a fast-paced hospital.

This Hospital is growing quickly and cater to the community in many levels. This position would entail, but is not limited to the following :

Job Summary :

Reporting to the Shift Manager of the assigned are, the Nursing Assistant (NA) performs duties under the direction of the Registered Nurse (RN) / Charge Nurse.

Responsible for providing high quality individualized basic care activities within the Nursing Assistant scope of practice.

Accepts assignments in hospital units for which qualified through completion of unit specific checklist.

Job Duties :

Responsible for always observing confidentiality practices regarding patients and staff.

2. Complete hospital approved training in managing patient behaviors and the de-escalation techniques.

a. CPI and complete initial and annual .edu on duties of the constant observer for self-harm patient and .edu for restraint and seclusion.

3. Demonstrate understanding of the duties on constant observer roles and responsibilities education sheet.

4. Demonstrate documentation on constant observer flow sheet.

5. Demonstrate use and documentation of physical environment checklist.

6. Demonstrate ability and understanding of reasons for always staying with patient and always keeping patient visible.

7. Demonstrate understanding of reasons for accompanying patient off unit for any tests / exams and visual observation of patient while bathing / toileting.

8. Demonstrate how to call for assistance if patient leaves or becomes physically aggressive.

9. Demonstrate ability and understanding related to removal of all harmful objects on their body that could be used by a patient to harm themselves or the observer (scissors, cell phones, sharp objects, stethoscopes, name badges, etc.)

10. For ED patients screened positive and under precautions, demonstrate understanding and ability of maintaining patients green safety scrubs for personal safety.

11. Demonstrate appropriate hand off with nurse while taking breaks or needs to be relieved for other personal needs.

12. always Maintains constant observation of patients and does not engage in other items such as reading, cell phone use, or any activity that could cause distraction away from the patient while performing constant observation.

13. Follows the multidisciplinary, goal-oriented care plan for each patient in collaboration with members of the health care team and

provides care based on that plan.

14. Adheres to EMC practices for pain management with specific consideration of patient's functional / development age, gender, cultural, and / or personal preferences.

15. Demonstrates appropriate use of restraints.

16. Ensures patient safety by assessing and proactively implementing safety measures with specific consideration of patient's functional and development age.

17. Competent in all routine skills within the patient care tech scope of practice including, but not limited to (1) assistance with activities of daily living, such as bathing, feeding, ambulating, toileting, (2) safe use of any medical equipment, (3) measurements of vital signs, weights, calorie count, intake and output, and (4) performing EKG's.

18. Assists in care of the patient during admission, transfer, discharge, and / or death. Performs ongoing observations of the patient and reports abnormal findings to RN.

19. Demonstrates the ability to provide care in emergency situations and follows established emergency.

20. Assist nurses in maintaining integrity of patient belongings.

21. Completes legible, accurate and timely documentation of all nursing care provided to patients.

22. Provides care and performs observations of all age groups of patients that demonstrate knowledge and consideration of age specific and cultural differences.

Modifies care to meet the age specific requirements of the newborn (0 to 31 days), infant (31 days to 1 year), pediatric (1 to 12 years), adolescent (13 to 17 years), adult (18 to 70), and geriatric (71 years) patients, including but not limited to, patient safety, and special physical and psychosocial needs.

23. Accepts assignments in hospital units for which qualified through completion of unit specific checklist. Additional responsibilities and skills will be dependent on completion of appropriate orientation and validation of competency.

24. Maintains a clean, orderly working environment (bedside stands, trash, suction canisters, pantry, blanket warmer, utility room, stuffs charts, and files information).

25. Help with any clerical duty the unit may need and respond to any call light that is needed.

Qualifications :

High school graduate or GED preferred

3 months skilled nursing or hospital experience preferred

Certified Nursing Assistant certification required

Current AHA BLS certification

Updated immunizations

Schedule Rotating Days 6 : 45a-5 : 15pm
