DescriptionReports to:Program/Department: Vida/SIDA Classification: Exempt DateSUMMARY:This person will be in charge of facilitating the TWIST and PROMISE interventions. TO PERFORM THIS JOB SUCCESSFULLY, AN INDIVIDUAL MUST BE ABLE TO PERFORM EACH ESSENTIAL DUTY SATISFACTORILY. THE REQUIREMENTS LISTED BELOW ARE REPRESENTATIVE OF THE KNOWLEDGE, SKILL, AND/OR ABILITY REQUIRED. Recruits and enrolls appropriate participants in the TWIST and PROMISE interventions.
Facilitates the TWIST and PROMISE interventions.
Collaborates with the Program Coordinator in the recruitment of the advisory group, which meets quarterly.
Assists clients in attaining their goals by identifying and locating community resources for clients and by making referrals to appropriate services both within and outside Vida/SIDA and the program.
Conducts community outreach, networking, and advocacy in appropriate social networks in the city of Chicago.
Follows TWIST and PROMISE guidelines to increase skills and self-efficacy with regard to protective behaviors.
Educates clients about HIV risk sensitized to personal risk.
Maintains program data and prepares session reports as required.
Meets regularly with the Project Coordinator and attends staff meetings as required.
Maintains client confidentiality and complies with any and all Federal, State, and City regulations as well as Vida/SIDA security and privacy policies intended to protect the security and privacy of individually identifiable health information.
Conducts online outreach through social media and dating sites to recruit clients and disseminate sexual health messages.
Prepares marketing materials for the program.
Follows up with clients and with referral organizations regarding client contact and progress with referral organization.
Contacts clients via telephone, mail, and/or face-to-face home and office visits to ensure adherence to program, and document via progress notes.
Creates and maintains client files and makes copies of all client documents.
Conducts pre- and post-intervention assessments of clients’ knowledge, attitudes, and behaviors regarding HIV risk.
Participates in appropriate trainings.
Performs other duties and functions as assigned.
This position requires compliance with all of PRCC’s written standards, including its Standards of Conduct, all policies and procedures and Regulatory Compliance requirements. Compliance will be considered as part of the regular performance evaluation.Supervisory Responsibilities

RequirementsQUALIFICATIONS:
Some post-secondary education required. Bachelor’s degree in health-related field preferred.
Bilingual English/Spanish preferred.
Must have at least 1-year experience working with the LGBTQ of color community and/or working with diverse communities.
Knowledge of HIV prevention, transmission, and harm reduction techniques.
Must be able to work evening, after-hours, and weekend hours within high-risk environments and communities.
Previous experience in street and/or community outreach preferred.
WORKING CONDITIONS: OSHA Category 2 – Normal work practice involves no exposure to blood, body fluids, or tissues, but exposure risk may occasionally be encountered in performance of assigned tasks.
