St. Francis House (SFH) seeks a Rapid Rehousing Stabilization Case Manager to provide direct support to homeless individuals who are preparing for and transitioning to housing. Using a Housing First approach, the Case Manager will ensure that program participants are connected to all services and support they need to obtain and maintain permanent housing. This role works as a team with integrated services at SFH to stabilize individuals in housing, income, and behavioral health.

They transform lives using a holistic approach to understand and address behavioral health, housing, and employment needs.
