We provide low-barrier emergency shelters, supportive services, and housing-first-minded rental assistance that helps individuals and families step out of homelessness and back into the community. We are seeking compassionate and empathetic individuals who are interested in making a difference in the lives of others and their community.
Job SummaryThe Permanent Supportive Housing Case Manager will work as part of our Housing Case Management Team to provide direct Case Management services to families who have moved out of homelessness and are living in housing throughout Salt Lake County. The Case Manager will help connect families with resources, and work closely with the Case Management Team, other agencies, and community partners to help support and facilitate families in meeting their diverse needs. Case Management duties will include time in the office, in families' homes, and in agency vehicles transporting clients throughout the county. Services will be provided with an emphasis on Trauma Informed Care and Housing First principles.

Duties And Responsibilities
  • CM will work with families/singles with long term rental assistance vouchers and assist in getting them stabilized in their housing and community
  • Focus on housing and strengths-based practices to meet the unique needs of each tenant
  • Be familiar with rental assistance funding sources
  • Be familiar with Trauma Informed Care and Housing First practices
  • Provide clients with program referrals and links to other community resources, and facilitate each client's access to these resources
  • Provide direct case management to families and singles, including: crisis management, ongoing assessment, goal setting, and problem solving
  • Assist clients in obtaining employment and or working on applying for Social Security benefits
  • Advocate and negotiate on the client's behalf with property managers
  • Assess children's needs to make appropriate community referrals, and liaise with schools as needed for supportive services
  • Assist tenants in getting on waitlists for Section 8, Public, and Subsidized Housing
  • Attend weekly staff meetings, case staffing, and additional meetings as assigned
  • Engage in daily collaboration with other departments, maintaining and promoting a healthy team environment
  • Keep accurate records, case management notes, and statistical data as assigned
  • Other duties as assigned

RequirementsEducation and Experience
  • Bachelor's Degree in a related field preferred
  • Experience working with a diverse population preferred
Skills and Requirements
  • Ability to work individually in a self-directed manner, and as part of a team

  • Strong interpersonal skills and ability to work with diverse populations
  • Excellent organizational skills
  • Strong computer skills and able to use Microsoft Office programs
  • Access to reliable transportation; personal vehicle helpful

  • Must have a clean driving record and be insurable on our agency policy. Driving will be a daily requirement

  • Ability to lift 25 pounds
  • Ability to stand and/or walk for at least an hour at a time

Originally established as the Travelers Aid Society in 1923, The Road Home changed its name in 2001 to better reflect our mission of helping people step out of homelessness and back into our community. The Road Home is a private, non-profit social service agency that assists individuals and families with children experiencing homelessness in Salt Lake County and along the Wasatch Front. Our Salt Lake Community Shelter and Resource Center and Midvale Family Resource Center shelters can be accessed 24 hours/day, 365 days/year. Priority populations like families with children, Veterans and those experiencing chronic homelessness are assigned case managers to facilitate transitioning out of shelter and into housing as quickly as possible. Case managers work with clients to develop a basic needs assessment and a short-term case plan. Once in housing, case managers continue working with clients to help them build the skills necessary to maintain stable housing, increase income, and ensure access to mainstream benefits. This includes conducting in-home visits to ensure participants are meeting the terms of their lease, providing referrals to community resources, and helping to identify and eliminate barriers that have prevented households from finding and/or maintaining housing.
