The Road Home has been a leader in the fight to end homelessness for 100 years (1923-2023). We provide low-barrier emergency shelters, supportive services, and housing-first-minded rental assistance that helps individuals and families step out of homelessness and back into the community. We are seeking compassionate and empathetic individuals who are interested in making a difference in the lives of others and their community.

Job Summary

The St. Vincent Resource Coordinator will provide trauma-informed, guest-driven case management services to individuals who are experiencing homelessness with the goal of helping them into a placement at one of the Homeless Resource Centers quickly and efficiently. The services provided include identifying and engaging with the most vulnerable individuals at our St. Vincent de Paul overflow site, building rapport, facilitating transfers into the HRCs, and completing a warm handoff between the HRC case management teams and the new guest. The St. Vincent Resource Coordinator will also work with the St. Vincent de Paul and shelter teams to identify common barriers faced by individuals who are not currently in a Homeless Resource Center and help with establishing strategies and procedures for addressing the needs.

Put “Housing First”/Collaboration with HRCs
  • Engage with the St .Vincent de Paul team to identify who is staying at that location and what the needs/barriers to accessing services are.
  • Conduct outreach and build rapport with the most vulnerable at the St. Vincent de Paul location.
  • Help the most vulnerable individuals staying at the St. Vincent de Paul location transition into the Homeless Resource Centers.
  • Assist guests in identifying barriers and connect them with appropriate resources in the community.
  • Assist our guests and their belongings to move over to the Homeless Resource Centers as necessary.
  • Assist in transporting clients to Homeless Resource Centers, including using public transportation, cab vouchers, the Advantage Services Shuttle, and using agency vehicles as necessary.
  • Strong belief in the Housing First model. Willingness to initiate and have ongoing conversations with guests about housing.
Managing a Case Load
  • Maintain an ongoing caseload of clients interested in seeking resources, assisting with establishing goals and eliminating housing barriers.
  • Engage with all guests at St. Vincent’s to connect them with resource centers, resources, and housing options.
  • Follow agency guidelines to create healthy boundaries with guests.
  • Daily collaboration with all departments promoting a healthy team environment.
  • Seeking out, engaging, and networking with community partners to find housing options and additional resources.
  • Attend weekly team meetings and contribute to case conferencing conversations.
  • Keep accurate records, case management notes, and statistical data as required and participate in special projects as needed.
Learn to Manage Crisis
  • Provide direct, trauma-informed case management services to guests, including crisis intervention, harm reduction, outreach, ongoing assessments, goal setting, creative problem solving, using a network of community resources.
  • Utilizing the training we provide to jump into high-stress situations in an effective and trauma-informed way
  • Assist with de-escalation and crisis intervention with guests in crisis.
  • Collaborate with the crisis clinician and others to find ways to support individuals with mental and behavioral health barriers.
  • Ability to accept direction and constructive feedback from Supervisor.
  • Ability to set and maintain good boundaries, using Trauma-Informed Care as a guideline.

  • Demonstrates awareness of and sensitivity to diverse populations.
  • Ability to contribute to the Agency's commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
  • Understanding that housing first, harm reduction, low barrier access to resources is best practice and essential to our mission at The Road Home.

Education and Experience
  • Experience working with diverse and vulnerable populations is preferred.
  • Lived, work or volunteer experience preferred OR a degree in a related field

Physical and Equipment Requirements
  • Ability to lift 25 pounds.
  • Ability to stand and walk for at least an hour at a time.
  • Ability to drive a vehicle.

Originally established as the Travelers Aid Society in 1923, The Road Home changed its name in 2001 to better reflect our mission of helping people step out of homelessness and back into our community. Case managers work with clients to develop a basic needs assessment and a short-term case plan. Once in housing, case managers continue working with clients to help them build the skills necessary to maintain stable housing, increase income, and ensure access to mainstream benefits.
