Job Title: Clinical Care Manager
Job Summary
The Clinical Care Manager plays a pivotal role in enhancingpatient health outcomes through advanced nursing processes, care coordination,patient education, and team communication. This position is responsible formanaging and optimizing the care of high-risk, acutely ill, and chronically illpatients, ensuring quality, continuity, and effective utilization of healthcareresources.
Key Responsibilities:
1. Care Management:
- Provide targeted,proactive care management to high-risk patients.
- Offer short-termcare management for patients post-hospitalization or ED visits.
- Facilitate smoothtransitions between care settings.
- Establish andmaintain patient relationships with primary care providers.
2. Clinical Support:
- Deliver directcare management, including patient education and coaching.
- Collaborate withhealthcare providers to set and achieve treatment goals.
- Monitor patienthealth status and adjust treatment plans as necessary.
3. Patient and Family Support:
- Performcomprehensive intake assessments.
- Address barriersto accessing healthcare and make appropriate referrals.
- Engage in patientand family education to promote self-management and lifestyle changes.
4. Data and Quality Management:
- Collect andreport data on quality indicators.
- Participate inregular staff meetings and interdisciplinary care coordination.
- Prepare monthlyupdates on care management activities.
5. Departmental Collaboration:
- Work harmoniouslywith all departmental and organizational staff.
- Promote smoothdelivery of services and participate in performance improvement activities.
- Utilizeelectronic software applications and manage related administrative tasks.
Essential Functions:
Patient Interaction: Engage with patients and families tocommunicate care needs and barriers.
Resource Management: Assist patients in obtaining necessarymedications, equipment, and community resources.
Team Coordination: Work with a multidisciplinary team toensure coordinated and effective care.
Education andTraining: Identify training opportunities and share best practices withhealthcare providers and staff.
Knowledge, Skills, and Abilities:
Expertise in population health and quality improvementtools.
Proficiency in MS Office Suite (Outlook, Word, Excel,PowerPoint).
Ability to maintain confidentiality and work with medicallyvulnerable populations.
Strong organizational, multitasking, and customer serviceskills.
Excellent written and verbal communication abilities.
Qualifications:
Experience: 3-5 years in healthcare, with at least one yearin acute care nursing preferred.
Education: Bachelor’s degree in Nursing (BSN/BAN) or relatedfield (BS/BA).
License/Certification: RN license in the State of Montana.
