FLORIDA DEPARTMENT OF HEALTH - OPEN COMPETITIVE OPPORTUNITY

Your Specific Responsibilities:

Plan, coordinate, implement, bill and monitor case management services for HIV positive clients through the Alachua County Health Department for Area 3/13.

Perform Medical Case Management functions for Ryan White (RW) as specified by current rules and regulations from the State of Florida, Medicaid and any other governing agency. Document performed medical case management functions in CareWare, such as writing progress notes, Comprehensive Needs Assessment, Comprehensive Care Plan and Acuity. Comprehensive Needs Assessments need to be completed on an annual basis or as needed. Comprehensive Care Plans are to be updated every 6 months with client involvement. Acuity scale assessment is to be completed on an annual basis or as needed. Paper charts are to contain service authorizations, dental treatment plan, signed release of information, change of information form, lab results, medical records and any correspondence concerning the client’s care. Serve as back up to other Medical Case Managers. Maintain accurate and current client data and information related to the specific programs on the computer using States approved programs such as HMS, ACCESS, Microsoft Office Word and Excel, CareWare and the Ryan White Portal. Will conduct face to face visits on each client at a minimum of once a year. If assigned to a clinic, will attend clinic as scheduled. Will assist clinic personnel (physicians/nurses) with activities to maintain clinic flow (faxing, making phone calls, etc). Will ensure that non- case managed client in clinic are aware of Ryan White services. Will ensure that required HRSA data is entered into CareWare. This includes but is not limited to client medications, CD4 values, Viral Load values, documentation of HIV medical care.

Coordinate services between approved vendors, health care providers, clients and/or clients advocates as outlined in current rules and policies by receiving prescriptions written by medical care providers, ensuring that funds are available with case manager quarterly budget; authorizing services with approved vendors. Submit appropriate paperwork for all vendor services authorized to billing clerk by established deadlines.

Complete EARS weekly. Attend and participate in staff meetings and training programs.

Performs other work as requested by supervisor.

Preforms other duties as assigned, this includes but is not limited to Emergency duty. During Emergency duty: works during and beyond normal work hours or days in the event of emergency operations. Functions required include but are not limited to Special Needs Shelters, Red Cross Shelters or other emergency functions.

At all times maintains confidential information such as social security numbers, medical information and other confidential data in a secure manner. Respects the confidentiality of related issues by not discussing with unauthorized staff. Per DOH Information Security and Privacy Policy 7—Confidential information, staff have the authority to take confidential information into the field to perform job responsibilities. Staff must adhere to Field Policy for Confidential Information. Ensures discussion held between staff cannot be overheard by unauthorized staff. Reports suspected breaches of security to supervisor immediately.

Required Knowledge, Skills, and Abilities:

Microsoft Systems and balancing budgets. Ability to communicate effectively and professionally, both verbally and in writing.

Ability to effectively manage workload and work well independently.

Qualifications:

Minimum – Highschool diploma/GED

Preferred – experience maintaining and tracking inventory

Minimum of 1 year experience with data entry in electronic medical records system and professional front office setting.
