OPS ONLY Duties and Responsibilities:

  • The primary responsibility of this position is to assist in the coordination and documentation of health education messaging, community outreach events and activities as outlined in the CHP workplan.
  • Assist in the coordination and documentation of outreach events and community spotlights submissions.
  • Disseminate information to the appropriate partners regarding, resources, events, grant opportunities, health care access and quality, best practices, etc. monthly.
  • Maintain documentation of requested and disseminated resources.
  • Attend and participate in the meetings and coalitions that serve the community.
  • Increase the number of community partners and/or collaborations in projects.
  • Update Coalition members on CHD health messaging projects, events, issues, partner recruitment, and data tracking.
  • Maintain a professional and effective working relationship with external organizations that address health disparities.
  • Maintain resource guide of local agencies and organizations.
  • Update FDOH-Nassau Community Health SharePoint Events Calendar
  • Conduct planning and preparation for our reach events. Prepare weekly outreach after action reports.
  • Attend conference calls, trainings, and webinars regularly for professional development.
  • Lead or co-lead at least 4 quarterly (CHD internal) meetings, activities or events.
  • Coordinate/participate in activities/events that support the improvement of resident’s health status.
  • Track priority area projects goals, objectives, activities in the FDOH ClearPoint tracking system and update data monthly.
  • Serve on one (1) internal committee such as Employee Wellness Committee, Workforce Development, Performance Management Council, Customer satisfaction or Safety committees.
