The Tandym Group is Hiring a Nurse Assessor (RN) Near New York, NY

A health services company in New York City is looking to add a licensed Registered Nurse (RN) to join their team as their new Nurse Assessor.
Responsibilities:
The Nurse Assessor (RN) will:
  • Perform initial in-home pre-evaluation of patients
  • Review consumer medical documentation and or health referral forms, as relevant to the case
  • Enter evaluation data into electronic evaluation form and transmit, as required
  • Document any concerns, conflicting information, other issues that surface during the evaluation process
  • Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan
  • Discuss with consumer on health care options, supports needed, service vendor options and waiver options
  • Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan
  • Collect quality review data and any required documentation to support outcome measurements and record case notes into CRM
  • Identify opportunities for health promotion and illness prevention
  • Perform other duties, as needed
Qualifications:
  • 2 years of Nursing experience or 1 year of Nursing experience with UAS in Home Care background
  • Graduate of an accredited Nursing program
  • NYS Registered Nurse (RN) license
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • 3 years of Human Services experience
  • Bachelor's Degree in Nursing
