Community Hospice of Northeast Florida is Hiring an Alivia Home Care - Home Care Aide Near Gainesville, FL

Under the direction and supervision of the Registered Nurse, the Home Care Aide provides personal care services and performs related tasks. The Home Care Aide is responsible for performing duties assigned in order to increase the level of comfort, maintain personal hygiene, and ensure a safe environment. As scheduled, the CNA will work in homes or other care settings as needed to provide services identified in care plans such as activities of daily living or household chores. This includes assisting with bathing, laundry, other daily living tasks, and accompanying patients to medical appointments.
Primary Responsibilities:
  • Assist with activities of daily living, provide personal care, and provide light housekeeping duties as needed and directed in the plan of care.
  • Provide emotional support to participants and their families.
  • Observe and report physical and emotional changes to the RN or designee.
  • Survey and observe any safety issues and report to the RN of designee.
  • May provide laundry and bathing duties as requested.
  • Report all complaints and grievances made by the participant to the RN or Administrator.
  • Treat all patients/participants fairly, with kindness, dignity, and respect.
  • Relay patient-related information efficiently and effectively to appropriate staff members.
  • Maintain up to date records using EMR system so problems, plans, actions, and goals are accurately and clearly stated according to policies, best practice, and regulatory requirements.
  • Successfully complete the annually required compliance and in-service training.
  • The Home Care Aide must be professional in appearance and verbally, being sure to always provide quality customer service to both the patient as well as their fellow colleagues.
  • Attend regular staff meetings as scheduled.
  • Display critical thinking skills to problem solve and manage patient and family issues according to policies and procedures.
  • Inform the RN or Administrator of unusual, problematic patient/family issues.
  • Participate in emergency disaster processes as defined by policy.
  • Demonstrate familiarity with policies/procedures and follows state and federal regulatory requirements.
  • Maintain all HIPPA and confidentiality of patients.
  • Follow all Policies, Procedures, and OSHA safety guidelines.
  • Maintain the confidentiality of all company procedures, results and information about clients and families.
  • Manage potentially infectious specimens with appropriate biohazard precautions and practices standard precautions.
  • Maintain a safe working environment.
  • Other duties as assigned.
Required Licenses/Certifications:
  • Current CNA certification in Florida or completion of a 40-hour Home Health Aide training program or successful completion of the Home Health Aide Competency Test
  • CPR Training from a certified provider
  • Current State of Florida Driver's license with driving record acceptable to company's insurance carrier; reliable transportation.
  • Current Auto Insurance Card
  • Current TB testing, immunization or declination for Hepatitis B and flu.
  • Level II background screening and drug testing
Qualifications:
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
  • High school diploma or GED equivalent preferred.
  • Reliable transportation that allows employees to travel between patient locations.
  • Furnish one or more active telephone numbers that enable management to contact the employee should an unexpected change of work location be required.
  • 6 months' experience providing care in patient homes preferred.
  • 1-year experience working with the frail or elderly population.
Knowledge of:
  • Knowledge of principles, practices, standards, and techniques of a home care aide
  • Knowledge of common safety hazards and precautions for a safe work environment
  • Knowledge of medical equipment use and maintenance
Skilled at:
  • Ability to communicate and establish rapport with patients/families.
  • Ability to handle challenging behaviors
  • Ability to react calmly and effectively in emergency situations
  • Ability to maintain patient confidentiality at all times
  • Skilled in establishing and maintain effective working relationships with patients, co-workers, medical staff, and the public
  • Basic Computer literacy
